This short self-assessment is designed to help you understand how prepared you and your family are to manage the healthcare side of caregiving at home.

You’ll find 10 questions that may seem detailed—but that’s intentional. Each one highlights a key part of navigating, coordinating, and documenting care effectively.

By the end, you’ll have a better sense of what’s working, where there may be gaps, and what steps you can take to feel more confident and in control.

There are no right or wrong answers—just an honest look at where you are today so we can help you move forward with clarity and support.

Kindly answer the following:


1. We/I share medical updates, care instructions, and follow-up tasks in a clear, consistent way so that everyone on our home care team stays on the same page.
2. We/I feel confident managing medications, referrals, insurance issues and understand when and how to use or share advanced directives when needed.
3. We/I understand my loved one’s care plan or multiple care plans and can explain them to others who are helping out.
4. We/I have a reliable system for organizing provider instructions, visit notes, and important medical documents.
5. For each doctor’s appointment, we/I know what to ask, how to summarize the visit, what follow-up steps are needed and how to share that information with our care team or other providers if needed.
6. When unforeseen incidents occur, like an adverse medication event or an unexpected trip to the ER, we/I feel prepared to act quickly and decisively, not overwhelmed, overreactive, or caught off guard. We/I can handle the situation and make informed decisions in the moment.
7. We/I have clearly defined roles in our care team, so responsibilities are understood and nothing falls through the cracks.
8. We/I are able to coordinate between multiple providers, specialists, or facilities—especially during transitions like hospital discharges.
9. We/I know how to track and communicate medical history including provider names, visit summaries, diagnoses, and past treatments—so everyone involved has a clear picture of care.
10. We/I don’t rely solely on the doctor’s electronic health record, we maintain our own up-to-date records to support better coordination, especially when switching providers or seeking second opinions.
