It helps you organize the medical side of care—appointments, paperwork, instructions—so you’re not overwhelmed or missing steps. By giving you a clear structure, this program is geared toward helping your care team to be more productive, keeps everyone on the same page, and improves coordination with healthcare providers—even when managing multiple conditions. Families feel more confident, more in control, and better equipped to stay on top of care.
Either way works. This program is designed for flexibility—your loved one can go through it themselves if they’re able, or you can take the lead as their health advocate. Many families manage it together, with one person keeping track of everything while others stay informed and involved.
Your doctor provides what’s known as condition management education—they diagnose, treat, and guide you through your specific medical issues. This education happens in the exam room and is delivered by a licensed healthcare professional.
Patient Better offers what’s known as healthcare navigation and care coordination education. This is the guidance most families never receive: how to organize records, manage appointments, coordinate care across providers, and prepare for what’s ahead. It’s the structure that supports condition management—the administrative and communication side of care that patients and families are typically left to manage on their own.
The Academy equips you with that structure—so you can stay informed, make better decisions, and keep your care team aligned.
The program is self-paced, so families can go through it as their schedule allows. Some choose to complete it together over a few weeks, while others take it one section at a time. The designated health decision-maker—often the power of attorney or the client themselves—typically leads the process and ensures the full program is completed.
Other family members or caregivers can be invited to complete specific lessons relevant to their role. For example, if a teen granddaughter is responsible for taking her grandmother to appointments, she might go through the appointment prep lesson to feel more confident and make that visit more productive.
Ultimately, this program helps the entire care team get on the same page, stay organized, and work together with more clarity and confidence.
That’s completely up to you. Some people simply use what they’ve learned to stay organized and confident in managing care. Others use this knowledge as a foundational staple in advocating for healthcare—for themselves and their loved ones—carrying it with them throughout their entire lives.
